Once you have booked your event, please complete this form to let me know about the basic details.

It is usually advised to complete the form nearer to your event to allow for any changes as you make plans.

MUSIC PLAYLISTS - you are welcome to provide anything from an extensive song list to a short list of favourites and you can also tick the music genres below and leave the rest to my experience.  You can create your lists on Spotify or iTunes etc and then send me a link via email or by copying it into the box on this form.

Please also feel free to request anything particular at any time during the event.

PARTY PHOTOS - if you have booked my Party Photos Service you are welcome to bring your own photos along on a USB drive on the day so they can be displayed on my screen during your event.

Photos play in alphanumerical order, so edit the photo file name and number them if you wish for them to be in a particular order
